TKE Medication Refill Guidelines

The patient must have a future appointment scheduled before being able to refill.

Medications we DO refill at TKE

Blood pressure medications

Potassium Binders

  • Veltassa

  • Lokelma

Sodium bicarbonate

  • lowers acid levels in the blood

  • treats low CO2 level

Transplant anti-rejection medications

  • Ex: prednisone, tacrolimus, mycophenolic, cyclosporine, etc.

  • Must have levels checked within the last 30 days before refilling immunosuppressant medications

famotidine (Pepcid)

  • Used to replace PPI

  • Treats acid reflux

Vitamin D (ergocalciferol or cholecalciferol)

  • Must have vitamin D checked within the last 6 months before refilling

calcitriol & cincacalcet

  • Must have PTH and vitamin D checked within the last 6 months before refilling

Diuretics

  • Ex: furosemide, bumetanide, torsemide, hydrochlorothiazide (HCTZ), chlorthalidone, metolazone, etc.

SGLT2 inhibitors (sodium/glucose cotransporter 2 inhibitors)

  • also called gliflozins or flozins
  • ONLY Jardiance or Farxiga

  • NOT Invokana

MRA (mineralocorticoid receptor antagonist)

  • spironolactone

  • finerenone (Kerendia)

Gout

  • allopurinol

  • colchicine (must be renally dosed)

Others (but not limited to)

  • calcium acetate (PhosLo), Velphoro, Auryxia: treats high phosphorus

  • tamsulosin, finasteride, dutasteride, etc.: BPH

  • oxybutynin

  • potassium chloride

  • sodium chloride

  • magnesium (must have magnesium level checked within last 30 days)

Medications we do NOT refill at TKE

Cholesterol medications

  • Most common: anything that ends in -statin

Proton pump inhibitors (PPI)

  • anything that ends in -prazole

Blood thinners

  • Eliquis, Warfarin, Xarelto, Plavix, aspirin, etc.

Other

  • Pain medications

  • Thyroid medications

  • Mood stabilizers

  • Erectile dysfunction (ED)

  • Diabetes medications

  • Weight loss 

  • Anti-nausea and/or appetite stimulant

  • Antibiotics 

  • Diflucan (at provider’s discretion): treats yeast infections

  • Vitamin B-12

  • febuxostat (Uloric):  prevents gout flare-ups but has a black box warning
